Recording Data - Nursing Assessment Form

Baseline Observations

Baseline Observations is expanded by default within the Nursing Assessment form.

To record any Baseline Observations:

  1. Select Options next to the heading you wish to record.
  2. Select Add .

  1. Input the required information.

  1. Select Save . .
  2. The data displays in a graph format on the Baseline Observations screen.

All Other Data

To record all other data on the Nursing Assessment form:

  1. Select the drop-down arrow next to the required heading.
  2. Select New.
  3. Input the required information.
  4. Select Save . .
